Intermountain Health seeks a Social Work Care Manager to deliver patient-centered care management within our Healthcare & Medical Services team. In this role, you’ll complete psychosocial assessments, develop individualized care and discharge plans, coordinate community and behavioral health resources, and support patients and families through transitions of care. You’ll collaborate closely with physicians, nurses, and allied health professionals to reduce barriers, address social determinants of health, and promote safe, high-quality outcomes. Conduct comprehensive psychosocial assessments for patients and families
Develop and implement individualized care and discharge plans
Coordinate community, behavioral health, and social service resources
Collaborate with an interdisciplinary team to support safe care transitions
Provide crisis intervention, counseling, and patient/family education
Address social determinants of health impacting care and outcomes
Advocate for patient needs and remove barriers to access and continuity of care
Document assessments, plans, and outcomes in the electronic health record
Participate in quality improvement and care management initiatives
Maintain compliance with regulatory, ethical, and organizational standards
