Position Details

• Position: Licensed Clinical Social Worker (LCSW)
• Schedule: Monday–Friday, 8:30 AM – 5:00 PM
• Contract Length: 8-week initial engagement
• Setting: Acute Care Hospital
• Float Requirement: Must be willing to float to sister facilities and other qualified departments as needed

Required Experience

• Hospice or Palliative Care experience required
• Experience conducting advanced directive discussions with end-of-life patients and families
• Hospital-based social work experience strongly preferred
• Experience with discharge planning, case management, and psychosocial assessments
• Candidates without hospice/palliative care background will not be considered

Required Education & Licensure

• Master of Social Work (MSW) from a CSWE-accredited program
• Active LCSW License required

Key Responsibilities

• Conduct comprehensive psychosocial assessments
• Provide counseling and emotional support to patients and families
• Develop and implement psychosocial care plans
• Coordinate discharge planning and community resource referrals
• Facilitate advanced care planning and end-of-life discussions
• Collaborate with physicians, nurses, case managers, and interdisciplinary teams
• Monitor patient progress and identify barriers to care goals
• Advocate for patients across the continuum of care
• Document assessments, interventions, and discharge plans in the medical record
• Support quality improvement and community networking initiatives

• Experience supporting patients and families coping with grief and loss
• Excellent communication and counseling skills
• Strong organizational and documentation abilities
• Knowledge of:
  • Motivational theories
  • Systems theories
  • Developmental theories
  • Group dynamics
• Ability to work effectively with diverse patient populations from neonatal through geriatric
• Basic computer and database proficiency
• Strong patient advocacy and care coordination skills

Documentation & Systems

• Experience documenting in electronic medical records
• Familiarity with Cerner and MIDAS+ preferred

Physical Requirements

• Frequent standing, walking, bending, twisting, and computer use
• Ability to travel between facilities when coverage is required

• Focus on psychosocial, emotional, financial, family, and community resource needs
• Must demonstrate strong collaboration, communication, competence, and patient-centered care
• End-of-life care expertise is a critical requirement for this role.
